bn: fix BN_DEBUG + BN_DEBUG_RAND support

Couple of updates to make this code work properly again;
* use OPENSSL_assert() instead of assert() (and #include <assert.h>)
* the circular-dependency-avoidance uses RAND_bytes() (not pseudo)
